根因：旧套续假走 /leave/saveExtend，官方提交体把定位放在**顶层**
address/latitude/longitude（由前端 locationData 展开），不使用 location 子对象。
此前续假表单直接复用请假的 location:{address,latitude,longitude} 结构，
服务端找不到顶层坐标，返回「请假定位参数错误」。
修复：BuildLeaveExtendForm 删除 location 子对象，改用顶层 address/latitude/longitude
（+ locationType），其余沿用请假体。依据：官方 home.*.js 中 saveExtend 请求体构造
（{...model, address, latitude, longitude, locationType, leaveId, extendReason, ...}）。
新增单测覆盖：顶层坐标存在、无 location 子对象、leaveId/extendReason 正确。
go build/vet/test 通过。
